How Do I Qualify for the VOTC, Inc. Care Coordination Program?
Each member/client will work with our Care Coordinator to determine if they are eligible for this program.
Once determined eligible, the Care Coordinator will work with each member to accomplish goals to overcome current barriers to a successful sober life.
The member/client must be enrolled in either Visions Outpatient SUD Treatment Program or Intensive Outpatient SUD Treatment Program to qualify for the VOTC, Inc. Care Coordination Program.
What Does the VOTC, Inc. Care Coordination Program Offer?
Medical Services/Referrals (Insurance, Primary Care Physician, Current Medical Concerns, Dental Care, and Hygiene services).
Mental Health Referrals (Trauma, Domestic Violence, Grief Loss, Coping Skills, and Mental and Emotional Support).
Social Services (Life Skills, and Recovery Support with a Recovery Coach).
Vocational/Educational Services.
Job Placement and Retention Services.
Community Connections (Children’s Services, Probation, Behavioral Health Tribal, and Court, Legal Aid, and Child Support Services).
Other Services: Transportation, Transitional Housing, Clothing, Childcare, and Parenting Education.
